What This Policy Covers

This policy covers HealthBASE’ treatment of personally identifiable information that we obtain when you visit our website for any reason except to use the HealthBASE services, which are subject to separate terms and conditions. During the registration process, we request and collect your name, the name of your organization, you role within your organization, and your email address, postal address, telephone and facsimile number, along with other information that we may request from time to time in order to identify which of our services may be of interest to you.

Access To Your Personal Information. In general, we make it easy for you to view the personally identifiable information we have collected from you in your user profile, where you can add, edit, or delete it as you see fit. You also can contact us (as explained in the first paragraph of this Privacy Policy) if you have any further questions about the collection, storage or use of your personal information.

2. Information We Do Not Collect.


Children. As explained further in our Terms of Service, you must be at least 18 to submit any personal information through our website. In addition, you must be 13 years of age or older to use our website in any way. We do not knowingly collect personally identifiable information about children.
